The Office of the Registrar, located in room 125, is the official record-keeper of your time here at NCC. From the time you register for your first class, to the end of your scholastic journey through graduation. The Registrar’s Office is also where you can request your official academic transcript, by submitting the Authorization to Release a Transcript form.

- Course Registration Form
If you are not eligible to register online, you will need to register with your academic advisor by using the Course Registration Form.
- Course Add / Drop Form
Submit this form to the Registrar’s Office to officially add or withdraw a course from your semester schedule. Academic advisor signature, required.
- Registration “Stoppers”
A helpful list defining why a course cannot be added to your schedule while registering online.
- Change of Major
Submit this form to the Registrar’s Office if you are looking to change or add a major to your academic record. Academic advisor signature, required.
- Change of Name and/or Address
Submit this form to change personal information, such as your name and/or address. Please note: All name changes require legal proof (i.e. Social Security Card, Driver’s License, Marriage License, etc.)

- FERPA: Directory Information Opt Out Form
By submitting this form to the Registrar’s Office, you are opting out of any disclosure of directory information. For more information, please click the link above that is entitled “Notification of Disclosure of Directory Information”
- Intent to Graduate Forms
- Spring 2012 – Degree
Seniors who expect to complete all academic requirements toward their degree program during the 2012 academic year, must file the Intent to Graduate form to the Registrar’s Office in order to be awarded.
- Spring 2012 – Certificate
Seniors who expect to complete all academic requirements toward their certificate program during the 2012 academic year, must file the Intent to Graduate form to the Registrar’s Office in order to be awarded.
- Summer 2012 – Degree
Seniors who expect to complete all academic requirements toward their degree program during the Summer 2012 semester, must file the Summer Intent to Graduate – Degree form to the Registrar’s Office, in order to be awarded. Please note: Summer graduates are not eligible to walk at the Spring 2012 graduation ceremony. However; they do have the option to participate in the Spring 2013 graduation ceremony.
- Summer 2012 – Certificate
Seniors who expect to complete all academic requirements toward their certificate program during the Summer 2012 semester, must file the Summer Intent to Graduate – Certificate form to the Registrar’s Office, in order to be awarded. Please note: Summer graduates are not eligible to walk at the Spring 2012 graduation ceremony. However, they do have the option to participate in the Spring 2013 graduation ceremony.
- Release of Student Information
Students must authorize in writing who they allow to be able to communicate with us regarding their student information. (i.e. parents, guardian, spouse, etc.)
- Replacement Degree or Certificate Request
Submit this form to the Registrar’s Office to request a replacement degree or certificate.
- Authorization to Release Transcript
In order to process a request, the Registrar’s Office must have in writing the authorization of the student. Please allow 2 business days for processing time.
